Before You Begin
1. Internship/Practicum Registration Form

This is the official course registration form for the college and MUST be completed within the drop/add period of the appropriate* term to receive credit. An instruction sheet is also provided.
*For those interning in the summer but requesting 1 fall credit, registration must take place within the drop/add period of SUMMER term in order to have the required supervision.

These are the forms that your site supervisor will use to evaluate you at mid-term and at the end of your internship. Be sure to find a time to discuss them with your supervisor.
2. Internship Journaling Guide

A guide to creating and maintaining a useful journal of your internship experience. Journaling may be required by your faculty supervisor.
