This is the official course registration form for the college. It MUST be completed and turned in to the Academic Records office within the drop/add period of the appropriate term to receive credit. An instruction sheet is also provided.

Complete this form at the beginning of your internship search and return to the career center to let us know what you're looking for.
Evaluations
Your internship supervisor will be asked to evaluate you twice - once and midterm and once just before finals week. Once they have submitted the evaluation, you'll receive an email asking you to review it. Watch for that email from careercenter@central.edu! Once you've reviewed your evaluation, the internship coordinator will send a copy to your faculty supervisor.
